Yes, it blinds his eyesight for a bit everytime he uses it. That’s the bad side of Mangekyou, including the need of great amount of chakra.

Also he uses the mangekyou sharingan like every episode he gets into a fight scene. Also when Jiraiya attacked Itachi and kisame, Itachi had to use one of his most powerful attacks just to get out of Jiraiya’s jutsu he even said he had to rest after that cause he weakened himself with only those 2 attacks. Just imagine if they had stayed Jiraiyas next attack would have killed them.
